Photodynamic therapy using methylated 5-aminolevulate (MAL-PDT) appears to have an effect on non-neoplastic skin diseases, for example acne vulgaris and rosacea, for which antibiotics are sometimes used, and a possible antibiotic effect of PDT has previously been suggested. It does, however, also cause local immunosuppression and post-treatment barrier defects, which may promote infection. At the same time, PDT-induced therapeutic skin damage is sometimes confused with secondary bacterial infection by non-dermatologists. The possible changes in bacterial flora associated with MAL-PDT were therefore studied in 47 patients undergoing treatment. Skin swabs were taken immediately before applying the MAL and instantly after light irradiation. Bacterial growth was identified in 18 cases. No statistically significant changes were seen, either in the specific species found or the estimated bacterial density on the skin surface. The observations do not support the notion that routine MAL-PDT affects the bacterial flora of the skin in a clinically significant manner. Therefore, the possible antibacterial effect of routine MAL-PDT is probably not the main explanation of its apparent effect on non-neoplastic skin disease

BACKGROUND: Erythromycin is a common therapy for acne and rosacea. A newer macrolide, azithromycin, offers superior tissue distribution and cellular concentration and is an effective oral anti-acne agent. Topical formulations such as erythromycin have been a major clinical therapy for acne. To date, no topical solution of azithromycin is available for the treatment of acne.
OBJECTIVE: To prepare a stable topical 2% azithromycin formulation that could be used in an acne clinical trial to determine the efficacy of topical azithromycin in treating subjects with acne vulgaris and acne rosacea.
METHODS: The study was divided into two phases. In phase I, azithromycin was prepared over a range of ethanol/water concentrations to determine solubility. The stability of a 2% azithromycin in 60% ethanol/water preparation was assessed by high-pressure liquid chromatography. The temperature, light, and pH dependence of the stability was also assessed. In phase II, a single center, randomized, double-blind, treatment-controlled study compared once-nightly application of topical 2% azithromycin versus 2% erythromycin. A total of 20 subjects with moderate inflammatory acne and 20 with rosacea were examined clinically at 0, 2, 4, 8, and 12 weeks for a 12-week period. Efficacy was evaluated with the Physician's Visual Analog Scale evaluation (PVAS), the papulopustule count, and acne severity rating (in subjects with acne).
RESULTS: In phase I, azithromycin was soluble in 60% ethanol/water. A 2% azithromycin in 60% ethanol/water solution maintained stability at room temperature for up to 26 weeks but at 37°C there was some decay (16%) at 26 weeks. The stability was greatest at pH 6.8 and was unaffected by ambient light exposure. In phase II, the number of inflammatory lesions decreased in both acne and rosacea subjects treated with 2% erythromycin (7.56, p=0.03 and 4.4, p=0.01, respectively). Azithromycin was not as effective for the treatment of rosacea. Both azithromycin (p=0.01) and erythromycin (p=0.03) treatment significantly reduced the inflammatory lesion count in acne vulgaris. No significant adverse events were identified in the acne group. In patients with rosacea, transient irritation occurred in five patients.
CONCLUSIONS: A 2% azithromycin in 60% ethanol/water solution can be prepared and is stable for at least 6 months at room temperature. The methodology and power of the study were adequate to identify improvement in acne vulgaris and rosacea. Though it appears the formulation of topical azithromycin was at least comparable with topical erythromycin, larger studies would be needed to determine whether topical azithromycin has any significant advantage over topical erythromycin.

At the end of 12 weeks, there was a 75% decrease in total scores (P < 0.001) and an 89% decrease in inflammatory lesion scores compared with basal values. Improvement continued during the 4 weeks after treatment.
Adverse effects were minimal and well tolerated in most patients.
Azithromycin is a promising agent in the treatment of rosacea with its few side-effects and good patient compliance.

Bacteria that normally and harmlessly live on the skin, called Propionni bacterium acnes, begin to grow in the hair follicle. These bacteria produce chemicals that cause inflammation. Inflammation is a response to disease or injury that is characterized by swelling, redness, heat and pain. There are a number of different types of lesions associated with acne. They are called cornedones, papules, pustules, and nodules or cysts. Comedones are simply enlarged, non-inflamed hair follicles that are plugged with oil and bacteria. A closed cornedo, also known as a whitehead, appears on the skin surface as small whitish bumps. An open comedo is also known as a blackhead because it looks black on the skin’s surface. The black coloration of an open comedo is not due to dirt, but discoloration of the sebum. Inflamed lesions vary in severity. Papules are more serious lesions than whiteheads or blackheads. Papules are inflamed comedones that appear as pink bumps on the skin and can be painful to the touch. More serious are pustules and nodules. Pustules are inflamed puss-filled lesions that resemble whiteheads, but with a red ring around the base. Nodules or acne cysts are the most serious of acne lesions. A nodule is a large, deep-seated, puss-filled, often painful lump. Acne with nodules often results in permanent scarring. Papulopustular acne also known as acne vulgaris is characterized by the presence of comedones and mildly inflammatory lesions. The severity of a case of papulopustular acne is determined by the ration of comedones to papules and pustules. The higher the numbers of inflammatory lesions that are present on the skin, the more severe the acne. Nodulocystic acne is more severe than papulopustular. Onychomycosis is a fungal infection of the finger or toe nails which is very difficult to cure.
Causes:

Acne vulgaris represents both a challenge to the treating dermatologist and a major concern for the patient. Conventional treatments have proved inconsistent with often unacceptable side effects and high rates of recurrence. Non-thermal, non-laser, phototherapy for acne with a combination of blue and red light has recently attracted attention. The present study was designed to assess the efficacy of this combination phototherapy.Methods: Twenty-four subjects, Fitzpatrick skin types II-V, with mild to severe symmetric facial acne vulgaris were recruited for the study. Subjects were well matched at baseline in terms of both age and duration of acne. Subjects were treated over eight sessions, two per week 3 days apart, alternating between 415 nm blue light (20 minutes/session, 48 J/cm2) and 633 nm red light (20 minutes/session, 96 J/cm2) from a light-emitting diode (LED)-based therapy system. Patients received a mild microdermabrasion before each session. Acne was assessed at baseline and at weeks 2, 4, 8 and 12. Results: Twenty-two patients completed the trial. A mean reduction in lesion count was observed at all follow-up points. At the 4-week follow-up, the mean lesion count reduction was significant at 46% (p = 0.001). At the 12-week follow-up, the mean lesion count reduction was also significant at 81% (p = 0.001). Patient and dermatologist assessments were similar. Severe acne showed a marginally better response than mild acne. Side effects were minimal and transitory. Comedones did not respond as well as inflammatory lesions. Conclusions: Combination blue and red LED therapy appears to have excellent potential in the treatment of mild to severe acne. Treatment appears to be both pain- and side effect-free.

Using antibiotics to treat acne for long periods may double the risk of throat infections, a study says. The University of Pennsylvania team found using antibiotics for more than six weeks increased the chance of a range of infections within a year.
Long-term use of antibiotics is a standard and commonly-used treatment for people with severe acne. A senior British GP acknowledged the risk of infection but said the findings were not likely to alter practice.
The US researchers used a UK medical database to compare the number of upper respiratory tract infections, such as the common cold, sore throat, tonsillitis and laryngitis, in people with acne taking antibiotics compared to those not taking the drugs.
Out of 118,496 people with acne surveyed, 72% received either topical or oral antibiotics for more than six weeks to treat their condition, the Archives of Dermatology reported.
Yes, there is a risk of infection, but that does not mean we should stop using antibiotics Within the first year, 15% of patients with acne suffered at least one infection. The odds of an infection developing in someone on antibiotics were about 2.15 times greater than in those not on the drugs.
Antibiotics increase the risk of infection by replacing "good" bacteria in the throat.
Lead researcher Dr David Margolis said: "The true clinical importance of our findings, in which patients and practitioners need to balance the risk of these infections with the benefits that patients with acne will receive from this therapy, will require further investigation."
Peter Swinyard, a member of the British Medical Association's GPs committee, said the risk was known, but would not change practice in the UK as the benefits of using antibiotics outweighed the negatives.
"Acne can be a major problem for teenagers and women. It can lead to breakdowns in relationships and loss of confidence.
"It is about time we acknowledged how serious it can be. Yes, there is a risk of infection, but that does not mean we should stop using antibiotics.

Scientists have cracked the genetic code of a bacterium involved in acne, an advance they say could lead to new treatments.
The German team from Georg-August University in Gottingen found genes that help the bacterium attack and destroy components of human skin.
Others were involved in triggering inflammation.
Knowing how the bacterium causes acne could offer new avenues of drug research, say the authors in Science.
The common skin problem affects up to 80% of young adults, but can affect older people too.

It usually starts in puberty when skin glands produce excess grease. Dead skin cells and grease block pores on the face, upper arms, upper back, and chest.
Researchers have known that the normally harmless bacterium Propionibacterium acnes, which is found on all adult skin, is implicated in acne.
This bacterium lives in the glands that secrete oil into hair follicles.
The German team sequenced the entire genome of the bacterium and identified 2,333 genes.
The function of many of these genes explained how the bacterium might trigger acne. Many were blueprints for enzymes that break down the skin.
Lead scientist Holger Bruggeman said: "We were astonished to see how many genes were involved in degrading the human tissue."
He said the bacterium lived off the products of this degradation.
New treatments
"The next step is to find a therapy specifically against those enzyme systems."
He said it was unclear why some people were more prone to acne than others, but said it might be down to the immune system.
"When the bacterium degrades the human skin the inflammation kicks in. We think the immune system of each person is different."
He said the reason acne is common in adolescence might be because this is a time when the skin produces more oils, which the bacterium feeds off.
Alison Dudley, chief executive of the Acne Support Group, said: "It's a really exciting development for future generations.
"We know that having acne can lead to suicidal thoughts and depression.
"To have a major breakthrough is very welcome for the future but, in the meantime, follow the acne support two-month rule. Try a treatment for two months and if you see no improvement go back to your doctor and try something else.
"There's something that will work so don't despair," she said.
