I used the Demodex shampoo a few months ago and yea!!! my hair stopped
falling out but about a month ago it started all over again and now it
is as bad as it was before I used the shampoo. I'm ordering more of
the shampoo and starting over again but is there a maintenance
program? How often should I use it to keep the little buggers away
since it is kind of harsh and drying to my scalp? Also can I mix it
with a little water before I put it on my wet hair?

Shampoo Demodex can be used once or twice a day, for best results, it should be used at night before bed. Mites are more active at night so it is better to use it before bed.
Apply to damp hair, massaged the scalp until the foam, leaving the head 2 - 3 minutes and rinse with warm water. Apply a second time massaging the scalp until the foam. Hold 3 - 5 minutes and rinse with plenty of water.
The average time you might need to get rid of the demodex mites is 1 - 1.5 month and even after the symptoms of demodex are gone, you should use the shampoo once a day for few more weeks to make sure that demodicosis is gone for good.
You should not mix it with water, and use the shampoo the way it is. It does not lather as shampoos with chemicals do but it has enough natural cleaning sabstances to clean hair.
You shouldn't use the anti dust mites spray on your hair if it has chemical ingredients in it. Chemicals suppress the immune system letting demodex mites to multiply.
